Exceptional Needs Services

  • The intent and purpose of this program is to assertively engage, case manage and to provide accommodation and personal support services to homeless Aboriginal people with high and complex needs linked to social and/or functional impairment arising from factors of co-morbidity. These may include diagnosed or undiagnosed disabilities, and/or chronic substance misuse.

How do we do this?

  • All clients are case managed
  • All clients have an allocated Case Planner to assess the client, establish a plan, monitor and review the plan
  • All clients are supported by Client Service Workers who implement the case management plan
  • Case conferences are convened every three months to include all relevant stakeholders to ensure that the client receives the best possible service to address client issues

Referral to ENS

All referrals to APOSS ENS must be made through Housing Support Program Exceptional Needs Unit.


HSP's criteria for referral include:

  • The client must be 18 years or older
  • Currently sleeping rough


  • Chronic homelessness history and is currently in secondary or tertiary homelessness


  • Has complex needs, which will be linked to some or all of the following:
  • Chronic alcohol abuse
  • Other drugs/substance use
  • Chronic poor health
  • Requires assistance with activities of daily living
  • Challenging behaviours
  • Risk to self or others
  • Significant multiple impairment
  • Significant social impairment
  • Not linked to specialist Mental Health Services
  • Not linked to specialist Disability Services
